Paw Hills Pet Hotel — Boarding Agreement & Liability Waiver

By signing below, I acknowledge and agree to the following terms and conditions:

  1. I certify that my pet is in good health and has not been exposed to any communicable diseases within the past 30 days.
  2. I understand that Paw Hills Pet Hotel requires current vaccination records for all pets. Vaccinations must include Rabies, DHPP (dogs), and FVRCP (cats) and must be valid throughout the duration of the stay.
  3. I authorize Paw Hills Pet Hotel staff to seek emergency veterinary care for my pet if necessary. I agree to be responsible for all veterinary costs incurred.
  4. I understand that while every precaution is taken, Paw Hills Pet Hotel is not responsible for illness, injury, or escape that may occur during my pet's stay.
  5. I agree to pick up my pet by the scheduled checkout time. Late pickups may incur additional daily charges.
  6. I understand that aggressive behavior may result in early termination of my pet's stay, and I agree to pick up my pet promptly if notified.
  7. I authorize Paw Hills Pet Hotel to use photos or videos of my pet for promotional purposes unless I opt out in writing.
  8. I agree to provide accurate and complete information about my pet's health, behavior, and dietary needs.

This agreement is effective from the date of signing and applies to all future bookings unless revoked in writing.
